Instructions for Completing Pesticide Testing Commission Form and Payment:

1. Since March 2017, our center has implemented the issuance of electronic invoices. If an invoice requires a tax ID, please provide the Unified Business Number in the payment information section. 

2. The sender must "personally sign" at the bottom right corner of the commissioning form. (Commissioning form download webpage link) 

3. Please photocopy the payment receipt and send it along with the samples. Alternatively, you can email, fax, or send via LINE to our center.Fax Number: 04-22854817 (Please call in advance to notify that a fax will be sent.)

4. Samples can be delivered in person or sent by mail

5. Testing Fees:

For the 410 pesticide residues listed by the Ministry of Health and Welfare, the fee is NT$5000.Additional fee of NT$500 applies for testing of dithiocarbamate pesticides (e.g., Furadan, Dithane, Maneb, etc.).
